KHC Annual Fundraising Begins

The Kentuckiana Health Collaborative is conducting its annual fundraising campaign.   All contributions are tax-deductible and are used to pursue the KHC’s mission to mobilize the community to improve health and well-being.   The KHC has four priorities of focus:  1) health care quality, 2) chronic disease, 3) obesity, and 4) smoking.  All current projects are aligned to support these priorities and are described in the 2011 KHC Project Description.

There are four levels of sponsorship you are asked to consider. Your participation as a sponsor allows your organization to be recognized for supporting a healthier community and will be prominently displayed on the KHC letterhead, website, and communication.  In order to guarantee maximum recognition, please complete the Donor Commitment Form  indicating your sponsorship level by June 10, 2011.

Sponsorship Level Contribution
Platinum $10,000 and up
Gold $5,000-$9,999
Silver $1,000-$4,999
Bronze $250-$999
