Join the Kentuckiana Health Collaborative on December 6, 2016 for a community forum about diabetes care in the region. Attendees will learn about the current state of diabetes in our community, gain insights from regional benchmark providers who reached levels of optimal care for their patients with diabetes, and hear the story of one patient who has overcome diabetes. This interactive session will demonstrate how positive deviance success stories can be disseminated and adopted to transform communities.
