From 2022-2023, the Kentuckiana Health Collaborative led the Diabetes Learning Collaborative (DLC) and Patient Voice Project – initiatives focused on how to reimagine diabetes care through multi-stakeholder collaboration. The DLC was formed to engage healthcare purchasers, providers, and payers together to implement interventions to optimize the prevention and management of diabetes and the overall health and well-being of persons with diabetes. Through multi-stakeholder collaboration, the DLC increased understanding and awareness of how different stakeholder types understand and address diabetes prevention and management, maximize each other’s action plans, problem solve for barriers that arise between their operations, and identify opportunities for partnership.
The DLC was a partnership between the Kentuckiana Health Collaborative (KHC) and the Kentucky Department for Public Health (KDPH). Funding was provided by KDPH through the Diabetes Prevention and Control Program through a grant from the Centers for Disease Control and Prevention, NU58DP006612. The McNary Group provided facilitation services.
The learnings of the DLC and the Patient Voice Project are detailed in the report, “Reimagining Diabetes Care Through Multi-Stakeholder Collaboration.” The report is now available to the public. This report provides a summary of this initiative to disperse the learnings among healthcare stakeholders and inspire similar shifts in understanding and action so that other healthcare stakeholders can reimagine how they support and deliver diabetes care.
