Join the second Community Health Forum with Dr. Marjie Harbrecht to learn how ALL healthcare stakeholders play a role in transforming the healthcare system.  With the realization that the US healthcare system is unsustainable in its current form, there is a tremendous movement across the country to redesign how healthcare is delivered and paid for.  One of the preeminent models being implemented and shown to be effective is the Patient Centered Medical Home (PCMH).  This presentation concentrates on what a PCMH is; what it includes such as team based care, EMRs/data reports, patient engagement, care coordination/care management, etc.  It also touches briefly on the importance of the Medical Neighborhood.
