Fourteen Hospitals in Kentucky Earn an “A” on the Spring 2025 Leapfrog Hospital Safety Grade

LOUISVILLE,KY, May 7, 2025 – The Leapfrog Group, an independent national nonprofit focused on patient safety, released its Spring 2025 Hospital Safety Grade on May 1, 2025. The biannual Safety Grade is an “A,” “B,” “C,” “D” or “F” assigned to all general hospitals in the United States based on their ability to protect patients from medical errors, accidents, injuries and infections. Driven in Kentucky and Southern Indiana (Clark, Scott, Floyd, and Harrison Counties) by the Kentuckiana Health Collaborative, the Grades give the community critical insight about local hospitals.

In Kentucky, 60 hospitals were graded, and 14 earned an A, 19 earned a B, 23 earned a C, 4 earned a D, and 0 earned an F.  

23.3% of hospitals in Kentucky earned an “A” grade, ranking Kentucky 36th in the country for overall percentage of “A” hospitals. This marked a setback from the Fall 2024 Safety Grades, where Kentucky ranked 22nd in the nation with 29.5% of hospitals receiving an “A” grade.

The fourteen hospitals earning an “A” grade were:

  • St. Elizabeth Healthcare – Ft. Thomas
  • St. Elizabeth Healthcare – Edgewood
  • Blanchfield Army Community Hospital
  • St. Elizabeth Healthcare – Florence
  • Harrison Memorial Hospital
  • Medical Center at Bowling Green
  • Baptist Health Richmond
  • Meadowview Regional Medical Center
  • Spring View Hospital
  • UofL Health Shelbyville Hospital
  • Baptist Health Deaconess Madisonville
  • Clark Regional Medical Center
  • Three Rivers Medical Center
  • T.J. Samson Community Hospital

In Southern Indiana, (Clark, Scott, Floyd, and Harrison Counties), two hospitals were graded. Each received a “B” grade.

For the first time, Leapfrog is spotlighting the 346 hospitals nationwide that not only achieved an A for spring 2025 but sustained the grade for more than two years. This select group, representing 12% of all eligible hospitals, is now highlighted on www.hospitalsafetygrade.org. Medical Center at Bowling Green is the only hospital in Kentucky that achieved this accomplishment.

The Leapfrog Hospital Safety Grade is the only hospital ratings program focused exclusively on preventing medical errors and patient harm. It is updated biannually in the fall and spring, offering the public a reliable, independent assessment of hospital safety. The Grades are determined by public, peer-reviewed methodology, calculated by top patient safety experts under the guidance of a National Expert Panel.
