
Community Health Forum: Reimagining Diabetes Care Through Multi-stakeholder Collaboration

Diabetes has been a significant issue in Kentucky for many years and continues to worsen, despite the availability of numerous evidence-based interventions and significant resources dedicated to preventing and improving outcomes of the disease. In 2022, the Kentuckiana Health Collaborative partnered with the Kentucky Department for Public Health to facilitate the Diabetes Learning Collaborative (DLC) an 18-month initiative formed to engage healthcare purchasers, payers, and providers together to implement interventions to optimize the prevention and management of the overall health and well-being of persons with diabetes. The formation of the DLC was intended to bring a collaborative lens to the work of multiple stakeholders and their efforts to improve diabetes prevention and management.
Join the Kentuckiana Health Collaborative virtually on June 14, 2023 from 8am-10:30am as we bring in experts on diabetes care and participants of the DLC to discuss the landscape of diabetes and diabetes prevention and management and the process, final learnings, and recommendations of the DLC.
