Community Forum: Achieving and Maintaining Blood Pressure Goals Through Technology

The KHC June Community Health Forum will explore how use of electronic health records and health information technology can improve provider outcomes and patient health outcomes related to the identification of undiagnosed hypertension and management of hypertension. The event will include information on the self-measured blood pressure monitoring (SMBP) program, an overview of the current data collection landscape, and next steps for organizations to take regarding this important area of focus.
