Community Health Forum: From Theory to Practice: Operationalizing Health Equity Efforts

Overview

Efforts to address health disparities have been accompanied by renewed vigor in recent years. Many of these efforts have centered around raising awareness and educating organizations and individuals on systemic injustices, structural and interpersonal racism, and health disparities. While there is continued work to be done on this front, there is also an imperative to act to not only understand these forces but address them through structured, measurable processes and objectives. Join the Kentuckiana Health Collaborative on December 10, 2024, to learn best practices from organizations representing different sectors of the healthcare system who have effectively operationalized their health equity efforts.

Learning Objectives

  • Understand organizational priorities and their “why” for eliminating health disparities 
  • Describe the process and outcomes of efforts healthcare organizations have taken to operationalize health equity 
  • Identify best practices and recommendations for other organizations to operationalize their own health equity efforts 
  • Discuss internal and external barriers and successes to implementing initiatives to close to close health disparities 
  • Share opportunities for continued growth in health equity efforts and a vision for the future
