Thank you for being a member of the Kentuckiana Health Collaborative (KHC). Our mission is to champion a more effective healthcare ecosystem through collaboration, insights, and education. We take a regional, systemic, and collaborative approach towards solving the complex health problems that face our communities. As a member, you are part of that effort.

Each year we invite members to share feedback on your membership experience and on what you would like to see from KHC going forward. This short survey takes about 7-8 minutes, and your responses are anonymous. Your input directly shapes our programming and priorities.
