KHC September 9th Community Health Forum Registration Open

Join the Kentuckiana Health Collaborative on Tuesday, September 9th, 2014 for our special two-part Community Health Forum with an exciting line up of Patient-Centered Medical Home speakers (speaker bios).

Part 1:  Patient-Centered Medical Home & an Accountable Care Organization Journey  

Join Dr. Steve Cleves, Internal Medicine/Primary Care, and Pam Coyle-Toerner, MHHA, CEO, Queen City Physicians for an interactive discussion of their journey and experiences along the way to successfully transforming into a Patient Centered Medical Home.  They will lead a discussion that looks at what works and what barriers there are to overcome in making the transformation. We will also discuss bending the cost curve as we are all affected by the cost of healthcare whether we are a provider, employer or consumer.

Part 2:  Patient-Centered Medical Homes Impact on Quality & Cost and the Greater Louisville Transformation Plan

Please join us as Marci Nielsen, PhD, MPH — the CEO of the Patient-Centered Primary Care Collaborative — speaks on Innovations in Health Care Delivery: Patient Centered Medical Homes (PCMH) & Accountable Care Organizations (ACOs).  Marci will share an overview of health system transformation trends as they pertain to medical homes and ACOs as a means to advance the Triple Aim of lower per capita health care costs, better patient experience of care, and improved population health. This session will highlight the evidence of PCMHs impact on cost and quality, highlighting specific case studies.  Marci will also describe proposed payment frameworks for medical homes and ACOs and discuss strengths and weaknesses of proposed payment models. Finally, Marci will address medical home transformation to include new workflow redesign for care teams.

As the Kentuckiana Health Collaborative strives to develop a plan that can drive the triple aim goals of better health, better care, and better value in Greater Louisville, the tenets of Patient Centered Medical Homes have been identified as key to transformation.  Dr. Marjie Harbrecht, CEO of HealthTeamWorks, will lead a discussion on the development of a Greater Louisville Healthcare Transformation Plan.
